Accommodation Letter Request Form

Students registered with the Disability Accessibility for Students (DAS) office will now use the Accommodate system for semester requests.  After you have been approved for reasonable academic accommodations, you will need to activate these each semester.

Please use the link below to login to Accommodate using your CCRI credentials. 

Login To Accommodate: Registered DAS Students

 

1. Select “Accommodations” then “Semester Request.” 

2. “Add New.” Then choose from the drop down menu to the current semester.

3. You may either choose from:

"Submit for All Accommodations", then click "Submit".

OR

“Review the Renewal.” , which allows you to customize which accommodations you need for each course, then click “Submit". 

You will see the Request has been submitted.  From here a staff member will review your request and your faculty will be notified of your accommodations via email.
