If you are under 24, unmarried and have no legal dependents you will also need the
following information for your parents* in addition to the information above:
- At least one parent needs to create an FSA ID here
- Parent(s) 2023 & 2024 Federal Income Tax Return (including schedules)
- Parent(s) 2023 & 2024 W2’s, 1099’s or form Schedule C
- Parent(s) Social Security Number, date of birth, marital status and date
- Amounts of any additional income parent(s) received such as alimony, child
support, social security, workman’s comp
* If your parents are separated or divorced, you will need the information from the
parent that provided primary support and custody.
