Instructions:  Complete this form, sign and date it, and return the form along with ALL the scholarship applications to:  Geraldine Peixoto, CCRI Foundation, Flanagan Campus.  Upon receipt of all the applications and the sign-off form, the Office of Institutional Advancement will process the scholarship award within two weeks and mail the check to the selected recipient.

2.  Does the selected recipient attend the Warwick Campus? Yes__ No__

3.  Is the selected recipient graduating from CCRI and transferring  to a four-year institution? Yes__ No__

4.  Is a copy of the selected recipient’s acceptance letter from the four-year institution attached? Yes__ No__

5.  Has the selected recipient demonstrated financial need? Yes__ No__

6.  Is the selected recipient’s G.P.A. 2.5 or higher? Yes__ No__

7.  Are two (2) letters of recommendation from CCRI faculty/staff attached? Yes__ No__

8.  Does the selected recipient meets all the scholarship eligibility requirements? Yes__ No__
