2014-2015 Academic Year

  1. Dependent Household Size Worksheet: This form verifies information concerning household size and number in college as reported on the FAFSA. This form must be signed by the student and parent.

  2. Independent Household Size Worksheet: This form verifies information concerning household size and number in college as reported on the FAFSA. This form must be signed by the student.

  3. Dependent Household Resource Worksheet: This form is utilized to verify untaxed income received in 2013 for both the student and parent(s).

  4. Independent Household Resource Worksheet: This form is utilized to verify untaxed income received in 2013 for the student and spouse (if applicable.)

  5. Child Support Paid Worksheet: This form verifies information reported on the FAFSA concerning child support paid in 2013. This form must be signed by the student and parent (dependent student only.)

  6. Asset Verification Form: The Asset Verification forms collects asset values based on information provided on federal tax returns.

  7. SNAP (Supplemental Nutritional Assistance Program, formerly known as Food Stamps) Worksheet: This form verifies information reported on the FAFSA concerning the receipt of SNAP benefits by a member of the student's household in 2013. This form must be signed by the student and parent (dependent student only.)

  8. Student 2013 Non Tax Filer Worksheet: This form is required if the Student and/or Spouse will not file and was not required to file a 2013 income tax return with the IRS. If income from work is reported, a W2 must accompany the worksheet.

  9. Parent 2013 Non Tax Filer Worksheet: This form is required if the Parent will not file and was not required to file a 2013 income tax return with the IRS. If income from work is reported, a W2 must accompany the worksheet.

  10. Re-Evaluation of Income worksheet:  The Re-Evaluation of Income form is used to provide a student with an opportunity to inform our office of changes in income after the FAFSA has been filed. Those changes include, but are not limited to, loss of employment, parent loss of employment (for a dependent student), or any other extenuating circumstances that result in a significant decrease in either taxable or untaxed income.

    This form is not required for all students. Students needing this form will generally be referred to it by an Enrollment Services Officer.

  11. Statement of Educational Purpose: This form certifies who the student is and that the federal student aid they may receive will only be used for educational purposes and for the cost of attending the school for the 2014-2015 year.

  12. Financial Aid Appeal Form:  The Federal regulations governing the awarding of Federal Financial Aid requires that a student be making Satisfactory Academic Progress. In the event that you fail to meet the requirements, you may appeal. You need to use this form to submit this appeal.
