Attention: All communications from the Financial Aid Office are sent to students through their CCRI email. You are able to view your financial aid information by logging into your CCRI account and choosing the “for students” tab.

*** Important Note: All loan requirements [Master Promissory Note (MPN), Loan Entrance Counseling and Loan Exit Counseling] must be completed at www.studentaid.gov.

Electronic Forms

In order to expedite processing for 2020-2021, all required forms must be completed and signed electronically online. The form links are provided below for the student and/or parent(s) to submit their paperwork.

    1. This form can be used as student-only, student/parent or parent-only for verification of assets.
  4. Student 2018 Non Tax Filer Worksheet
    1. This form is required if the Student and/or Spouse will not file and was not required to file a 2018 income tax return with the IRS.If reporting income earned from work, 2018 W2's must be submitted.
  5. Parent 2018 Non Tax Filer Worksheet
    1. This form is required if the Parent and/or Parents will not file and was not required to file a 2018 income tax return with the IRS. If reporting income earned from work, 2018 W2's must be submitted.
  6. 2018 Federal Tax Transcript/Return Upload process-Student
    1. Allows the student and or spouse to upload Federal tax return transcripts using a personal computer or mobile device. Please note: When attaching documents, please choose Upload ONLY. The fax option is not operational for CCRI purposes.
  7. 2018 Federal Tax Transcript/Return Upload process-Parent
    1. Allows the parent to upload Federal tax return transcripts using a personal computer or mobile device. Please note: When attaching documents, please choose Upload ONLY. The fax option is not operational for CCRI purposes.

  9. 2018 Student W-2 Upload Process
    1. This document must be completed if the student must submit W-2 form(s)

    1. This document must be completed if the parent(s) must submit federal W-2 form(s)
  11. 2018 Student Verification of Non-filing Upload Process
    1. This form is required if a student and or spouse did not file a 1040 for the tax year 2018. This document provides proof that the IRS has no record of a filed tax return.
  12. 2018 Parent Verification of Non-filing Upload Process
    1. This form is required if a parent did not file a 1040 for the tax year 2018. This document provides proof that the IRS has no record of a filed tax return.

    1. Please note: This form must be signed and stamped in the presence of a Notary and mailed in with a copy of a valid government issued ID. This form will not be accepted via email and/or fax.
  2. Statement of Educational Purpose -In Person
    1. Please note: Student must appear in person at the Financial Aid Office with a valid government issued ID. This form will not be accepted via mail and/or fax.
