 Human information:
supraspinatus (rotator cuff)
origin:  medial 66.7% of supraspinous fossa
        of scapula
insertion:  greater tubercle of humerus and
        capsule of shoulder joint
nerve:  suprascapular (C5 and C6)
action:  abduction of arm

This is one of the four rotator cuff muscles.  It is the only rotator cuff muscle that is an abductor of the arm.  The above drawings of the origin and insertion might help visualize this information (red is the origin, blue the insertion).
