Step 3: Hiring Forms
Each type of Monthly Payroll employment has its own authorization form as follows:

Lecturers: Monthly Payroll Authorization (Non-Teaching Work) (monthly timecard) |

Adjunct Faculty: Adjunct Faculty Information Form |

Non-Credit Instructors: Monthly Payroll Authorization (Non-Credit Teaching) (monthly timecard) |
| Guest Lecturers, Speakers or Event Participants: Honorarium Payment (one-time payment) |
In addition to the respective authorization form listed above, each new Monthly Payroll employee, regardless of category, must complete the following forms:
| 1. Employment Eligibility Verification (I-9) with copies of 2 forms of identification as defined on the I-9 form. |
| 2. CCRI Criminal Background Check (BCI) |
| 3. Attorney General's Criminal Background Check (obtained from the R.I. Attorney General's Office) |
| 4. Drug-Free Workplace acknowledgement Form |
| 5. Bar of Claims - identifying previous Worker's Compensation injuries, if any. |

State Employment Application (CS14) (Answer questions 1 & 5 only and signature on 2nd page) |
| 7. W-4 - to establish income tax deductions |
| 8. Direct Deposit (optional) (Not available for Honorarium payments) |
