Step 3: Hiring Forms

Each type of Monthly Payroll employment has its own authorization form as follows:

AUTHORIZATION FORMS
Lecturers:
Monthly Payroll Authorization (Non-Teaching Work)  (monthly timecard)
Adjunct Faculty:
Adjunct Faculty Information Form
 
Non-Credit Instructors:
Monthly Payroll Authorization (Non-Credit Teaching) (monthly timecard)
 
Coaches or Faculty Stipends:
Special Monthly Authorization (Non-Hourly Reimbursement)
Guest Lecturers, Speakers or Event Participants: Honorarium Payment (one-time payment)

In addition to the respective authorization form listed above, each Monthly Payroll employee must complete the following forms listed below.

Please note:
1.  Honorarium payment employees must complete an I-9, Bar of Claims, and Drug-Free forms; no other forms are needed.
2.  *If a monthly payroll employee is rehired within 1 (one) year of his/her last appointment or already works for CCRI on the biweekly payroll, the employment forms are not required although a new authorization form may be.
 

REQUIRED FORMS
FOR NEW OR RETURNING* EMPLOYEES
 
1. Employment Eligibility Verification (I-9) with copies of identification as defined on the I-9 form.
2. Drug-Free Workplace Acknowledgement Form
3. Bar of Claims - identifying previous Worker's Compensation injuries, if any.
4. W-4 - to establish income tax deductions
5. Direct Deposit (optional) (Not available for Honorarium payments)
6. Campus Domain Account Application - needed to create a personal email account
7. Pre-Employment Consent Form -
A national background check will be conducted by HireRight, Inc. once the new employee completes this form.
