Instructions for a Non-Teaching Authorization Form

PART I
- This section must be filled out in its entirety by the applicant. The applicants signature is mandatory. Without this signature, the authorization will not be processed.
- Please make sure that the
applicant has completed the following forms and attached to the
authorization:
- W-4
- I-9 (copies of identification must be attached to this form)
- Bar of Claims
- Drug-Free Workplace
- BCI Check

PART II
- This section must be filled out in its entirety by the Dean/Department Chair/Director.
- Banner #’s can be found on the Human Resources web site under ‘CROSSWALKS’.
- Hourly rates can be suggested by the Department Chair/Director. Approval of hourly rate is determined by the Human Resources Office. A list of hourly rates can be found on the HR Monthly Authorizations PDF* file.
- Period of employment cannot exceed one year. All authorizations end on or prior to June 30th. At this time a new authorization can be filled out for the new fiscal year beginning July 1st or after.
PART III
Signature approval is in the following order:
- Department Chair/Director
- Dean (If applicable)
- Divisional Vice President
- Human Resources
- Business Office
- Vice President for Business Affairs
PLEASE NOTE THE FOLLOWING:
- Please allow ample time (2-3 weeks) for approval.
- Applicant cannot begin work until the approval process is completed and BCI check has passed.
- Upon approval, the department chair / director will receive a completed and fully signed copy of the authorization with a timecard attached; the employee and Payroll Department will receive a copy of the authorization form as well.
