Purpose: To
recognize the sophomore with the highest scholastic honors in the Clinical
Laboratory Technology Program (formerly Medical Laboratory Technology).
Application Deadline: April 1
Processing Procedure Allied Health Department Chair appoints scholarship review committee.
The Allied Health Department Chair completes Scholarship Sign-Off form and forwards it and all applications to the Office of Institutional Advancement. At the same time, the Allied Health Department Chair contacts Jean Lovett in the Office of Student Affairs for the Class Night Program Brochure.Allied Health Department Chair orders CLT Pin from CCRI Bookstore and plaque from Emblem & Badge. These bills are sent to the Office of Institutional Advancement for payment.
Upon receipt of all materials, CCRI Foundation sends congratulatory letter to the selected recipient with a copy to the Allied Health Department Chair. Copies of the letter plus a copy of the application form are sent to:
